Become a part of our caring community and help us put health first -The Home Health Social Worker provides individuals, families, and groups with the psychosocial support needed to cope with chronic, acute, or terminal illness. The Home Health Social Worker work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action.The Home Health Social Worker services include advising family care givers, providing patient education and counseling, and making referrals for other services. May also provide care and case management of interventions designed to promote, health, prevent disease, and address barriers to access to healthcare. Bachelor's degree in Social Work, and state licensure required. Understands department, segment, and organizational strategy and operating objectives, including their linkages to related areas. Makes decisions regarding own work methods, occasionally in ambiguous situations, and requires minimal direction and receives guidance where needed. Follows established guidelines/procedures.Use your skills to make an impact -Typically requires Bachelor's degree or equivalent and less than 5 years of technical experience
