Job DescriptionAveanna Healthcare is seeking a skilled Licensed Practical Nurse (LPN/LVN) to join our team of dedicated healthcare professionals. As a Private Duty Nurse, you will play a vital role in providing high-quality care to patients in their own homes, working closely with our patients and their families to develop personalized care plans that meet their unique needs.Responsibilities

• Implement physician-prescribed care plans and coordinate with our multidisciplinary team to ensure seamless care delivery.
• Conduct thorough patient assessments, identifying areas for improvement and developing strategies to enhance patient outcomes.
• Administer medications, treatments, and therapies as prescribed, ensuring accurate documentation of all care provided.
• Provide education and support to patients and their families, empowering them to take an active role in their care.
• Communicate effectively with our clinical team, patients, and families, ensuring timely and accurate exchange of information.Requirements
  • Current, unrestricted LPN/LVN license in the state of practice.
  • Current CPR certification.
  • Minimum 6 months of experience as a licensed nurse in a clinical care setting, with a strong preference for home health experience.
